The National Rosacea Society offers a "Triggers Checklist" tablet, a 25-sheet pad that conveniently combines the Rosacea Triggers chart and the Rosacea Diary into single tear-off sheets. The front side of each sheet features a list of common triggers, lifestyle and environmental factors that may cause flare-ups, while the reverse side contains a diary form to aid in monitoring these potential triggers. Patients simply make copies of the diary side to monitor their rosacea over the course of several weeks to identify and avoid individual triggers.
To receive a Triggers Checklist tablet of 25 sheets by U.S. Mail, fill out the Materials Request Form.

consultation with a qualified physician. The Society does not evaluate, endorse or recommend any particular medications, products, equipment or treatments.
